There's no threshold you have to reach. If worry or panic is getting in the way of things you want to do, that's a reasonable time to talk to someone.
Mostly a conversation. We talk about what's been happening, what you've tried, and what you'd like to be different. You don't need to prepare anything.
It varies from person to person. We'll set goals together and review progress regularly, so you always know where things stand.
